2008-09 Scholarship Application
STUDENTS APPLYING FOR CITY OF SALEM COMMUNITY COLLEGE ACCESS PROGRAM (SCCAP) FUNDS ARE NOT ELIGIBLE TO RECEIVE SCHOLARSHIPS OFFERED IN THIS PACKET – FOR INFORMATION ON SCCAP FUNDS, PLEASE VISIT VIRGINIAWESTERN.EDU/SCCAP OR CALL 540.857.7312.
We encourage you to apply online. If you apply online, you will not have to make copies of your essay for submission. You will however need to provide to the Foundation office a copy of your signed tax return, and submit a transcript by May 23, 2008.
INSTRUCTIONS FOR SUBMITTING PAPER APPLICATION
- Please print the following documents:
- Submit the application, in its entirety, to the Foundation office by 4:00 p.m., May 23, 2008 (please follow the instructions on the printed application).
- To apply for a Foundation scholarship, you must have a minimum cumulative GPA of 2.0, be a high school graduate or have a GED, AND be enrolled in a minimum of six (6) credits, unless the scholarship you are applying for requires you to be enrolled full-time. Full-time enrollment is defined as being enrolled in a minimum of 12 credits for students not in a Health Technology program. Full-time enrollment in a Health Technology program is defined as being enrolled in 10 credits.
- Required supplemental materials, such as transcripts, recommendations, copy of your signed tax return, and materials required for specific scholarships must be received in the Foundation office by 4:00 p.m., May 23, 2008.
- Submit your original application with one (1) copy of your signed tax return AND five (5) copies of your one-page essay. THE FOUNDATION WILL NOT MAKE COPIES FOR YOU. The Foundation must also have one copy of your transcript or GED, if applicable, and your recommendations by May 23, 2008.

- Please review the online application in its entirety before completing.

- After clicking Submit Application, it is imperative that you print a copy of your application (there is a link on the thank you page that leads you to a way to print the submitted application) to keep as a backup should the Foundation not receive your completed online application.
