A Vaccination Clinic will be provided by Kroger Health on October 13th from 9:00 a.m. to 11:45 a.m. in the Fishburn Hall HR Training Room. Free flu, shingles and pneumonia vaccines will be available for employees. To sign up for an appointment, click this link or call Laura Carper at extension 77962 to reserve a time. Once you’ve scheduled your appointment time, print out and fill out the consent form here. When you arrive, be sure to have a copy of your filled out consent form and a printed copy of your insurance.
