Faculty and staff should use the form below to request access to campus during this COVID-19 closure. Requests should be made at least 24 hours in advance during normal business hours, and will need to be approved by your supervisor.
You will be notified by email whether your request has been approved or denied. On approval, Facilities Management staff and Campus Police will be notified, and may perform random checks to verify that the requester stays in the specified locations at the specified dates and times.
If your request spans multiple days and there is a change in your health status, you must notify your supervisor immediately and stay away from campus until further notice.
