WellAdvantage will be on the college campus Friday, September 13th from 9 a.m. to 1:15 p.m. to conduct the biometric screenings for eligible persons. Screenings will be conducted in the Human Resources Training Room on the ground floor of Fishburn Hall.
As a reminder, biometric screenings must be completed between August 1 and October 31 either onsite or at a participating LabCorp, or have been done at a physician’s office between April 1 and October 31, to count toward premium rewards for the second half of the plan year. You must have had a completed health assessment to qualify for the premium reward, but it is not required should you wish to participate in the biometric screening.
Employees and non-Medicare retiree group participants who are enrolled in COVA Care, COVA HDHP or COVA HealthAware health plan, their spouses and adult dependents are eligible for the screenings. To learn more, visit: http://www.dhrm.virginia.gov/hbenefits/BiometricFAQs.pdf.

- Log-on using your user ID and password that was created when you took the health assessment.
- Under “Welcome to MyActive Health” click, “Click here”.
- Review the fields, confirm and complete any blank fields,
- Read: 1) Terms and Conditions, 2) Patient Consent Form, and 3) Privacy Statement.
- Click both radio buttons above “Save and Continue”.
- Click “Save and Continue”.
- Under “Select Your Screening Option” click “Click to Continue” next to “On-Site”.
- Next to “State”, select VA from the drop down menu:
- Next to “City”, select Roanoke.
- Scroll down to September 13, 2013 and click “Check Availability” next to VA Western Community College.
- Select a time.
- Click “Register”.
- You will receive an email registration confirmation.
On the same website, you will be able to retrieve forms for the biometric screening for use at LabCorp or your physician’s office.
